Pharma Giles: School For Scandal...



This spoof of the recent Cupcake Controversy that I helped spawn (I know, seems strange for a Kitty to care about Cupcakes, since I don't eat much sugar), is very funny. I am so glad PHARMA Giles returned to blogging. His/her sense of humor is priceless. Thanks for this :)

Can you believe that Patty Murray and Maria Cantwell BOTH voted against re-importation of medications from Canada? Is this because they represent a State, Washington, that borders Canada? Did PHARMA get to these two liberal Democratic women? They both campaigned for health care reform, listening to the populous, working class roots, and working for the voice of families.

Their voting seems contrary to their campaign promises. Does anyone care? I wish that every person who can not take their prescribed medications due to the cost, and would like their PHARMA costs halved, PLEASE write your Senators.

Check out the cool pictures on this posting. Those big Cats are awesome! This was an exciting and sad day for this old Black Cat. The blogging late into the night, was mistaken for a wild woman rage, my bad. But the content was valid. John Mack felt that Chris Truelove and I were battling out the Pink Cupcake Marketing. However, Chris and I agreed on more points than we disagreed. And she (and I think John Mack) are willing to listen to NON ranting, heart felt feedback on DTC and other PHARMA campaigns.

Also today, along a Rost-type plot, the job description for my replacement was placed, without telling me, and in the middle of negotiations. I only found out because the email was sent to me by my "pal." That is the way they announced my "resignation" from that position. Given, that I have not signed the letter of resignation, and that I was in on going talks, and that they SCREWED my staff (that is bad), and they have been sued for this very reason before.

You, John Mack, ended up laying down, and playing with me on my own level, just like the Husky dog in the aforementioned video below.

Since I would like to have this 5 minutes to tell this TOP PHARMA Marketing CEO my humble opinions, and I want to do it repectfully and in a way that encourages a discussion of these very important issues. It is my most positive of intentions, to help improve PHARMA marketing, because I support PHARMA industry in its current incarnation, about the same way I support the Bush Reign in the U.S. That is, I believe in the Country and the Office, but not its current practices, policies and ethics.

I am praying that Research Girl and other experts in the theories of communication and their interaction with Marketing Principles will speak up by commenting on Pharma Blogosphere about this, or on this page. I think hearing this information from other Women and Cancer Survivors or their Family Members would also help Dr. Mack, better formulate to the PHARMA companies that he is involved with, more articulate feedback than I could every give.

If you have a BEEF against direct to consumer (DTC) PHARMA CURRENT marketing in ALL media, and in your workplace, please comment on Dr. Mack's blog, or my blog, or ANY of these marketing blogs. If you believe as I do, that these companies advertise in a less than optimum way, and you can offer CONSTRUCTIVE feedback, not rants (like I unfortunately tend to do- follow my words not my example :)) this is a RARE chance to be heard.

DID YOU FORGET THAT ALL YOU WANT TO DO IS DRINK CLEAR FLUIDS DURING CHEMO? DID YOU FORGET HOW SICKENING THE CLOYING SMELL OF REFINED SUGAR IS TO SOMEONE WHO FEELS NAUSEOUS?

They're also all men.

Quite frankly, if I were a breast cancer patient and some cheerful people are handing me flowers and cupcakes on my way out of chemo, I'd be very happy about that (and having had a breast cancer scare in the past, this is not a topic far from my mind at times). I'm also enough of a cynic to understand that the flowers and cupcakes are not being given out just to make me feel happy. I realize that other people might not think the same way, though, and might ask their doctor about treatment with Arimidex even if it's not right for them."

MAYBE YOU WOULD, BUT WHEN IF I WAS LEAVING CHEMO, I BELIEVE I WOULD FIND THE DISPLAY DISTASTEFUL FOR SO MANY REASONS. IF YOU ASK, I WILL BE GLAD TO LIST THEM.

"But I don't think this initiative is truly tacky. Truly tacky is the tale that I heard once of an initiative by a sales representative from a top 10 pharmaceutical company - I am leaving out the company name and the name of the drug to protect the innocent in this admittedly anecdotal tale. If the tale is true (it was passed on to me from someone at the University of Pennsylvania Center for Bioethics), then this beats pink cupcakes all out. It seems that the rep went into the doctor's office with a tarp, a beach backdrop, and a sunlamp. She spread sand on the tarp, stripped down to a bikini, and sunbathed. The sign on the backdrop read, "[Our SSRI] is like a day at the beach." Allegedly this was a hit with the doctor, and the rep's idea, published in the department newsletter, got her a nice cash bonus that month."

THIS IS THE TYPE OF STORY THAT TEENAGERS USE WHEN THEY WANT TO JUSTIFY THEIR BAD BEHAVIOR. A SCENARIO MIGHT SOUND JUST LIKE THIS SUB PAR ARGUMENT: "MOM... PETER IS ALSO A BAD BOY, HE DID X, WHICH IS SO MUCH WORSE THAN THE Y I DID, SO PLEASE DON'T GROUND ME.. OR WHATEVER." THIS IS PURE BULLSHIT. JUST BECAUSE A POOR WOMAN WITH NO BOUNDARIES AND AN INTENSE FEAR OF FAILING AT HER HIGHLY INTELLECTUAL DRUG REP JOB, BEARS HER BOOBIES TO SELL MEDICATIONS, DOESN'T MAKE PRETENDING OT BE A PATIENT ORGANIZATION OF MOTHERS, WHEN YOU ARE REALLY A PHARMA COMPANY PROMOTING YOUR MEDICATIONS A FAIR AND ETHICAL MARKETING PRACTICE. FRANKLY IT SHOULD BE BROUGHT UP TO SANCTIONS.

IT IS NOT SO MUCH THE REFINED SUGAR, WHICH WOULD BE UNDESIRABLE FOOD TO PASS OUT FOR HEALTH REASONS ALONE (THINK DIABETES) OR THE FLOWERS, ALTHOUGH, ONE WOULD THINK THAT A LIVE PLANT WOULD BE MORE APPRECIATED THAN A DEAD FLOWER; RATHER IN MY HUMBLE OPINION, THE TACKINESS THING ABOUT THIS MARKETING IDEA GONE WRONG, IS THE MIS-REPRESENTATION OF "MUM" AS AN ORGANIZATION THAT IS NOT REALLY PHARMA, BUT ONE OF CAREGIVERS AND FAMILY MEMBERS. THIS SUBTLE DECEPTION IS WRONG, AND A BIG BANNER SHOULD HAVE TO FLY OVER THE GOODIE TABLE SAYING "AZ SPONSORS... THE MAKERS OF YOUR CHEMO...."

ON THE OTHER HAND, A MORE GENTLER, KINDER AZ MARKETER MIGHT DECIDE THAT A CAMPAIGN WHERE THEY DONATE 1000 TO EACH CHEMO PATIENT'S CHARITY OF CHOICE, OR A FREE MAKE-OVER, OR A FREE WIG CONSULATION, OR A FREE FOOT MASSAGE OR A FREE POPSICLE OR DESIGNER WATER OR JUICE WOULD BE SO MUCH BETTER IN THE LONG RUN.

DO THE MARKETERS REALLY THINK THE CONSUMERS ARE THIS STUPID? I HAVE YET TO MEET ANYONE, OF ANY LEVEL OF EDUCATION, WHO THINKS POSITIVELY OF THE RECENT MARKETING OF THE MAJOR PHARMACEUTICAL COMPANIES. THE BLUE PILL, THE GAS, THE DEPRESSION, THE PAIN, THE GIBBERISH ADS, THE ALLERGY ADS, AND MOST OF THE DTC MARKETING IS GOING TO BACKFIRE, I PROMISE. Second (or is it third?) NIMH Study Shows Treating Depression Does Not Lead to Better HbA1c's


"Improvements in depressive symptoms don't lead to better control of glucose levels, according to a new report.

Many people with diabetes also feel depressed, which is thought to interfere with their ability to regulate their blood sugar levels well. Dr. Richard S. Surwit, from Duke University Medical Center, Durham, North Carolina stated that, "While clinical depression should be treated in all patients, treating depressed mood (non-clinical depression) in patients with type 2 diabetes as a strategy to improve glycemic control is not effective."

Surwit and associates investigated whether 16 weeks of cognitive behavior therapy would affect control of blood glucose levels in 28 patients with type 1 diabetes and 62 with type 2 diabetes.

Over a one-year period, changes in depression symptoms did not have an effect on A1c levels -- a measure of glucose levels over the long term -- or fasting blood glucose levels, regardless of the type of diabetes, the researchers report in the medical journal Psychosomatic Medicine.

Even among the subgroup of 17 patients with clinical depression, the researchers note, there was no evidence of an improvement in HbA1c levels as depression improved.

The results were "somewhat of a surprise," Surwit said. However, he and his associates point out, "Thus far, the evidence of a link between depressive symptoms and glycemic control has been modest at best, and our current findings raise further doubts about this hypothesized relationship."

If anyone reads this and wants the PDFs for the ARCHIVES of General PSYCHIATRY paper, where this lack of efficacy was first seen (although the depression treatment was PHARMA and problem solving therapy), or actually the first NIMH trial where PHARMA depression medications were ineffective in changing blood glucose, let me know. I intimately know all those studies. Why don't they give up this hypothesis in the light of 3 negative, albeit published in top journal studies??? Inquiring minds want to know? Perhaps because the P.I's are benefiting greatly from PHARMA?

Posing as Pals, Drug Reps Sway Doctors' Choices

CHICAGO (Reuters) Apr 24 - As much as doctors would like to deny it, subtle attention from friendly drug sales representatives can have a big impact on what drugs they prescribe, according to two U.S. studies published on Monday.

"Physicians underestimate their own vulnerability. They think they are smarter... but they are not trained in recognizing this kind of manipulation," said Adriane Fugh-Berman, a Georgetown University Medical Center researcher and co-author of one of the studies.

Fugh-Berman teamed with Shahram Ahari, a former drug representative for Eli Lilly and Co., who now works at the University of California, San Francisco's school of pharmacy.

Their study, which appears in the Public Library of Science journal PLoS Medicine, details the elaborate methods used by drug company sales representatives to make friends and influence drug sales.

"Reps scour a doctor's office for objects -- a tennis racquet, Russian novels, '70s rock music, fashion magazines, travel mementos or cultural or religious symbols -- that can be used to establish a personal connection with the doctor," Fugh-Berman and Ahari wrote.

"A friendly physician makes the rep's job easy because the rep can use the 'friendship' to request favors, in the form of prescriptions.

"Physicians who view the relationship as a straightforward goods-for-prescriptions exchange are dealt with in a businesslike manner. Skeptical doctors who favor evidence over charm are approached respectfully, supplied with reprints from the medical literature and wooed as teachers," they wrote.

Sales representatives also ingratiate themselves by lining up paid speaking engagements for doctors and arranging educational grants to those who frequently prescribe their drugs.

The study comes as drugmakers are smarting over the public revelation this month from an AstraZeneca drug sales representative, who said in a unauthorized newsletter to staff: "There is a big bucket of money sitting in every office. Every time you go in, you reach your hand in the bucket and grab a handful."

An AstraZeneca spokeswoman said the manager was fired and the company was looking into the incident, which she said violates a core value of serving patients.

QUICK VISIT, LASTING EFFECTS

Another study found that even a brief visit by a drug sales rep could have a powerful impact.

The study analyzed surveys done by a market research firm that chronicled a doctor's intention to prescribe the epilepsy drug gabapentin during the period of 1995 to 1999, when it was sold by Warner-Lambert under the brand name Neurontin.

Pfizer Inc., which acquired that unit in 2000, paid a $240 million fine four years later for illegal promotion of the drug for unapproved uses such as migraines or pain.

The surveys involved 116 visits to 97 doctors.

They found that after 46 percent of the visits, the doctors said they intended either to prescribe gabapentin more often or to recommend it to colleagues more often.

"The remarkable thing is how effective a very brief visit by a drug representative -- most often less than five minutes -- can be in influencing physicians' choices to use a drug for an unapproved indication," Dr. Michael Steinman of the San Francisco Veterans Affairs Medical Center said in a statement.

Besides free drug samples, salespeople often bring gifts, lunch for the doctor or office staff, new pens and coffee mugs. "The doctor feels subtly, even subconsciously, indebted to the representative," Steinman said.

Black Kitty Can NOT Save the World

My friends keep telling me, Kitty, YOU can not save the world. I know intellectually that is true; its the emotional part that gives me grief.

Yesterday I met a brilliant, yet troubled man, who was the age I was, when I gave birth to my son. Young. He was obviously of high intellectual curiosity, claiming to speak several languages, being a technical student and using his creativity in clothing design. We had a wonderful chat, and he said some very nice things to me. But what was concerning to me was that this young man, in the past couple of weeks was ingesting: alcohol, cocaine, methamphetamines, marijuana, psychedelic mushrooms, LSD, Seroquel, Lithium and Depakote. This made me feel very uneasy and raised the little hairs on the back of my neck.

MEOW...... Kitty wanted to nuzzle the disoriented and not totally in reality young man and lick him back to a semblance of sanity. However, who is THIS Kitty to say he is not sane? According to him, he is living independently, has friends and family who he sees regularly, goes to school and works. This sounds like the boy is functioning. That is all that really matters... in this old Kitty's mind. QOL - quality of life... so this kid.. doesn't need saving from me.. unless, of course, these very powerful chemical and natural substances interact one day.. in an unexpected manner... I pray for that young male Kitten... to drop the chemicals and keep the herbs.... Peace and Love... Domo Arigoto...

White Kitty Vs. Alpha Husky Dog....Go Kitten Go...

This little kitty should not be judged by its color, even if she is 'white.' She is one brave feline and tries to put the Husky Alpha Dog in its place and defend its territory. However, IF the Husky wanted to, he/she could end the life of the little white kitty in a moment. But, thankfully, the Husky is the picture of compliancy and kindness to the bitty kitty. Now, if this was mapped into this BLACK Kitty and the Huskies she has to deal with daily, they would be chasing her around.. then sniffing her, then ignoring her. Such is the life of a Black Kitty, who has about a quarter of the spunk as this ying to my yang baby cat.

This video is a very good example of a consensual power exchange between two mammals. At some level, the kitten KNEW that Husky would not harm her, and at some level, I am totally assuming that the dog found the kitty amusing, young, and full of spunk. But the real power, is in the exchange of power. The dog lets the cat be the boss, even though the dog knows full well it is really in charge.

Pfizer's First Female Whistleblower: Illegal Marketing of HIV Drug

Health Care Hitman - Dr. Peter Rost breaks another big story. Jane Roe (aka) claims that Pfizer used the HIV sales force to illegally promote the drug to doctors before approval. The drug is called Maraviroc, and it will be the first CCR5 receptor antagonist on the market.

Rost talked to Roe, and her answers are very cogent and articulate with respect to the role of Pfizer in marketing a drug before it has been approved by the FDA. This is very illegal, yet the medical director of the HIV unit, who has no HIV clinical experience himself, along with a drug rep, went to MD offices to try to sign up patients for "trials" of the medication.

I find it amazing and 60 Minutes-like in reporting the fact that Dr. Rost has copies of the SLIDES for the presentation that was given to MDs on the sly. YOU CAN DOWNLOAD THEM. The Powerpoint is full of graphs and cool pictures. The graph here shows that the viral load is significantly decreased in comparison to placebo. Its also true that being open about your sexual orientation and HIV status lowers Viral load and CD4 count, and its drug-free.

This looks like SALES to me, however, tt is my opinion, that Pfizer needed to UP their N (number of people successfully taking the drug) and that, rather than advance sales, is what drove Pfizer to supposedly break the law. No matter the reason, the deed was wrong.

WHAT is UP with Pfizer these days? Are they on Crack?

It seems as though they are acting like meth or crack addicts by greedily breaking all the rules to infuse themselves with their favorite green happiness... the dollar.

Yup.. Uh Huh... WORD

Where IS the LOVE?



Looking at my last month's worth of posts, I am in a Kitty Query Mode. What is it that I am trying to say with this blog? Which parts of me are feeling expressed, and which parts are being stifled?

I think that the Devil is in the splitting of blogs in order to express the multi-facets of our interests and personality. I am very multidimensional in personality and nature. The cat family is known for these characteristics: They trust few, eat where they are fed, they give love on their terms only and they can draw blood in a fraction of a second.

I have reached out to many people in my search for personal health, work satisfaction and creative outlets. When this Kitty starts meowing, growling, and hissing about these topics and the state of the globe, people RUN don't walk away and don't look back.. until they are at a safe distance.

I don't blame them. Wild cats are the Queens of the Jungle. They always have been, and they always will be, if MEN do not kill them into extinction. Certainly, MEN have tried to kill this Black Kitty, literally and figuratively. Several women also. This is the nature of me, the beast, the queen of my jungle.

Having lived many years among humans, I am keenly aware of dynamics that exist between people, and between society and their representatives. I don't feel represented well, by any of my current relationships with people or states. I don't know whether this is because I am ill, or because I am multi-dimensional, or because to put it into Dyer terms - "I have not yet found my match within my relationships, work for money, creative outlets, and my government."

It is for these reasons, that I have left my country, in both spirit and flesh. I intend to become a Canadian. My family knows this. I can no longer support my representatives at a State or Federal Level. I can no longer tolerate it.

I am also a scientist, a mother, and an outsider. There has never been any one place in my life where I fit perfectly. I don't know the answer. I wish I did.

In the past months, I have contacted off line just about every blogger for which I could find an Email address. I offered up myself, to each of them, in support of their charitable mission of blogging for truth, justice, and choice in PHARMA land, and in health services research. My almost 30 years of experience working with the top PHARMA academic partners was essentially of no real interest to anyone, but me of course :)

It is for this reason that I am going to go it alone now. I am not going to hide behind the magnificent ALPHA DOG Bloggers anymore. They are intellectually stimulating, informative, timely, and generally most of them seem like really great people you would love to have to dinner. I just HOPE with all my heart, that they stop fighting. This Kitty tried to run around their legs.. making them chase HER instead of each other, but alas, their fighting for timeliness and ALPHA-ness has driven several very important bloggers out.

I don't think Peter or John or Ed or Jim or Phillip or CP wanted to do this. I don't think the wonderful Health Related or other composite or summary type blogs wanted to do this either. I just think, everyone should take a deep breath... and ask themselves this glorious sunny weekend (ok.. I am in a great place):

Two persons identifying themselves as New York police officers interrupted a 9/11 Truth demonstration on a public sidewalk in front of the new WTC 7 Building to intimidate free speech, stating "Larry [Silverstein] doesn't want to hear it," before accusing We Are Change founder Luke Rudkowski of having a bomb and that his cell phone was "a gun."

This video is VERY disturbing. Everyone should watch it, to understand what is going on RIGHT NOW in NYC. Our personal freedoms are at risk. Does anyone care? Will this story even get covered in our National Media.

Hearing the cops taunt the activists, well, it confirmed my worst fears.

This is not a belated April Fools' Day story in BusinessWeek :

Eli Lilly and Co., which built its reputation on insulin and antidepressant Prozac, launched its first prescription medicine for pets on Friday.

The company will market Reconcile, a beef-flavored chewable fluoxetine tablet that helps treat dogs for separation anxiety, in the United States.

I knew this was coming. My Kitty Vet was prescribing Prozac for Kitties over 5 years ago. He got in trouble (with who, I don't know) and was not allow to prescribe it, or Interferon for Kitties any more.

I think this is where PHARMA is going... to the dogs. Us CATS are too damn smart to be bothered with these Doggie drugs. In my opinion, Pets should BE like Prozac (theoretically not literally): They should make us feel less depressed and not give us side effects that we find intolerable. My boys.. Simon and Theo.. they are the Kitty loves of my life...

I think the REAL study that is screaming to be actualized, is a trial comparing head on, the psychiatric disorders in the OWNERS of PETS who ask for DOGGIE PROZAC in contrast to matched dogs and owners who would never think of asking for DOGGIE PROZAC . Is asking for Reconcile a projection of the owner's mood state? Is it Münchhausen by Pet?? Is it a disparate attempt to call to the attentions of a doctor (albeit a Vet) the owner's inability to "bond" with their pet. Or just maybe... your pet actually thinks you are sub par.. and considers its life... well... depressing.
